Hours spent in front of a screen, hand constantly on the mouse, shoulders stiffened in one position – and then comes that unpleasant pain that creeps from the wrist through the forearm all the way to the shoulder. It's called mouse shoulder, and it affects an increasing number of people who spend most of their day at a computer. The good news is that there are effective exercises and habits that can alleviate this problem or even prevent it altogether.
Mouse shoulder is not just a trendy term. It is a collective term for the overloading of muscles, tendons, and nerves in the shoulder, neck, forearm, and wrist area, caused by repetitive one-sided movement – typically operating a computer mouse. According to the World Health Organization, musculoskeletal disorders caused by work are among the most common occupational diseases of all. And the office environment is one of the biggest culprits in this regard.

The most effective exercises for mouse shoulder that can be done even in the office
Regular exercise is the fundamental pillar of both prevention and treatment of mouse shoulder. Physiotherapists recommend incorporating short exercise breaks every 45 to 60 minutes of computer work. It doesn't have to be anything demanding – even a few minutes of targeted exercises can make a big difference.
Neck and trapezius muscle stretching is among the most important. Sit up straight, slowly tilt your head toward your right shoulder, and gently pull it down with your left hand. Hold for 20 to 30 seconds and feel the pull along the left side of your neck. Repeat on the other side. This simple exercise releases the trapezius muscle, which tends to be chronically shortened and overloaded when working with a mouse.
Another very effective exercise is shoulder rotation. Sit up straight, arms loosely at your sides, and slowly roll your shoulders – first forward, then backward. Ten repetitions in each direction are enough to improve circulation and release tension in the joint capsule. It may seem simple, but performing this exercise regularly can significantly reduce pain.
Chest muscle stretching is another step that many people overlook. When working at a computer, the chest muscles are constantly shortened, while the muscles between the shoulder blades are overstretched. Stand in a doorframe, rest your forearms against the frame, and slowly lean forward until you feel a pull across your chest. This exercise helps restore the natural balance between the front and back of the torso.
For releasing the wrist and forearm, stretching the flexors and extensors works excellently. Extend your right arm in front of you, palm facing down, and use your left hand to bend it downward so that your fingers point toward the ground. Hold for 15 seconds, then bend the hand in the opposite direction, fingers pointing up. This exercise directly targets the muscles that are most stressed when operating a mouse.
We must not forget shoulder blade exercises. Sit up straight and try to draw your shoulder blades together and downward, as if you wanted to tuck your shoulder blades "into your pocket." Hold for 5 seconds and release. Ten repetitions of this exercise strengthen the muscles between the shoulder blades and help maintain proper posture, which is the foundation of mouse shoulder prevention.

For those who want to go further, strengthening exercises targeting the rotator cuff – a group of four muscles that stabilize the shoulder joint – are also suitable. Exercises with a light resistance band or small dumbbells under the guidance of a physiotherapist can significantly improve shoulder stability and reduce the risk of recurring problems.
An overview of basic exercises that can be performed directly at the work desk:
- Head tilt – trapezius muscle stretch, 20–30 seconds on each side
- Shoulder rotation – 10 repetitions forward and backward
- Shoulder blade retraction – 10 repetitions, hold for 5 seconds
- Forearm stretch – flexors and extensors, 15 seconds each
- Chest muscle stretch – in a doorframe, 20–30 seconds
Exercise alone is not enough, however, if the conditions causing the pain do not change. It is therefore important to pay attention to workplace ergonomics as well.
Ergonomics and lifestyle as the foundation of long-term relief
Exercise is effective, but without the proper setup of your workstation, it is a battle against windmills. An ergonomic workstation can significantly reduce the load on the shoulder, wrist, and entire spine. The foundation is the correct height of the desk and chair – the elbows should form approximately a right angle and the forearms should rest freely on the surface. The screen should be at eye level so that the head is tilted neither down nor up.
The choice of mouse also makes a big difference. A vertical mouse, which allows the hand to be held in a more natural position – as when shaking hands – significantly reduces the rotational strain on the forearm. Similarly, a mouse pad with a gel wrist rest helps prevent overloading of the tendons. These small investments in ergonomic accessories pay off many times over in the long run.
Regular movement throughout the day also plays an important role. Research published in the professional journal Journal of Occupational Health shows that short movement breaks every hour have a demonstrably positive effect on reducing musculoskeletal disorders in office workers. It is therefore not enough to exercise only in the morning or evening – the key is to interrupt long static positions throughout the entire working day.
The influence of overall lifestyle is also significant. Adequate hydration keeps muscles and tendons supple. A deficiency of magnesium, which manifests as muscle cramps and increased muscle fatigue, can contribute to pain. Quality sleep allows tissue regeneration. And stress – that quiet but powerful enemy – causes people to unconsciously tense their shoulders and neck, which multiplies the strain even further.
If the pain persists even after introducing regular exercise and ergonomic adjustments, there is no need to give up. Physiotherapy offers a whole range of methods – from manual therapy and dry needling to ultrasound – that can help even in chronic cases. It is important not to wait too long, as the longer the problems persist, the more difficult they tend to be to treat.
Mouse shoulder is not an inevitable fate for everyone who works at a computer. It is the body's signal that it needs more movement, better conditions, and a little more attention. And that is precisely where the good news lies – change is possible and it is in each of our hands. All it takes is a start, even if it means just five minutes of stretching a day.
